Employers/Small Business Partnerships:

Seabreeze DPC offers exclusive small business plans designed to provide affordable, high-quality healthcare access for your team. To qualify as a business partnership, enrollment must include eligible employees — though owners and their families are welcome to join as part of the plan.

  • 1-4 employees: $72 per employee per month. Enrollment fee waived.

  • 5 or more employees: $65 per employee per month. Enrollment fee waived.

  • Dependents (spouse/partner or children under 26) may also enroll under business plan. Adult dependent (age 18+) is the same price as an employee membership. Minors (age 12-17) are $50 per month.

Medical Weight Loss:

  • With DPC membership: $100 for initial visit, $75 for follow up visits.

  • Without DPC membership: $125 for initial visit, $100 for follow up visits.
